How to work with your dialysis care team

You are the person who sees every session; your care team sees the results. Good outcomes come from telling each other what the other cannot see.

What to know

  • Keep a session log — it turns "I have been feeling tired" into data your nephrologist can act on.
  • Report access changes, alarms and symptoms as they happen, not at the next monthly review.
  • Come to reviews with questions written down; consultations move quickly otherwise.
  • Ask for the reasoning behind prescription changes. Understanding why makes adherence much easier.

Talk to your care team
