Documentation and record keeping at home

Your session log is the clinical record of your treatment between monthly reviews, and it is what makes prescription changes evidence-based.

What to know

  • Record for every session: pre and post weight, blood pressure, session length, ultrafiltration volume and any alarms.
  • Note symptoms — cramps, dizziness, headaches — and when in the session they occurred.
  • Log access appearance and any needling difficulty.
  • Bring the log to every review, and photograph it as a backup.
