Using patient portals and telehealth

Remote review works particularly well for home dialysis, where the treatment data comes to the clinician anyway.

What to know

  • Telehealth suits routine reviews, results discussions and medication adjustments.
  • Have your session log, weights and blood pressure readings ready before the call.
  • Access problems, new symptoms and physical examination still need an in-person visit.
  • Keep digital and paper backups of your prescription and recent results.

Talk to your care team

This topic is part of Patient Rights, Advocacy & Resources, one of ten modules in the Trivita Learning Hub. If anything here contradicts what your own nephrologist or dialysis nurse has told you, follow your care team — they know your prescription, your access and your history.

Trivita's dialysis nurses answer our care line 24 hours a day on 055 242 1070, including for people who are not yet our patients.
